If the boys and girls make a bad choice outside of the classroom (P.E., Music, Art, Library, recess, cafeteria, on the bus, etc.) they will receive a "Broken Arrow" form and they will need to remove one of their feathers from their pocket once they return to the room.

As the boys and girls make good choices, they will receive "Caught Being Good" tickets and stickers. The stickers will be worn home and the tickets will be placed into a grade level box at morning meeting. Mrs. Rushing will pull one ticket out of every grade level box and those boys and girls will have the opportunity to walk to the store to receive a 'free icee'.

Interim Reports go home during the middle of each nine weeks for those students who are doing unsatisfactory work. Please contact the office to set up an appointment with your child's teacher.

Students who are assigned after school detention failed to follow the school rules and procedures. Detention is served on Tuesdays and Thursdays from 3:40 p.m. to 4:40 p.m. Please sign and return this form immediately.

If a student removes all of their feathers but the red one from their head band, he or she will loose their recess. A loss of recess form will be sent home.

Please keep this card up to date with current telephone numbers, addresses, and health information.

Each child must have parent or guardian permission to have their picture published in the newspaper or on the school web site.

Each grade level plans educational field trips during the year. Participating students must have a permission slip signed before leaving campus.

Students receive this report for failing to follow school rules and procedure. Please sign and return this form to your child's teacher. Please contact the office to schedule a conference to discuss these behaviors.
